This online form provides the Pre-health Professions Advising Office with student contact information and a list of letters of recommendation students anticipate to receive. Students must assure that the $20 application fee has been submitted to the Pre-health Professions Advising Office by the time students would like their letters of recommendation processed. - Request to send Letters of Recommendation Form
The student prompts the Pre-health Professions Office to send letters of recommendations to their institutions of interest by completing this form. - Student Waiver Form (PDF, 46K)
This form is to be filled out by the student and given to the recommender. The applicant gives permission to release information contained within the letter by completing this form. The student may elect to waive or restrict the right of access to their letters of recommendation with this form.
