Doctor of Pharmacy

  • Two letters of recommendation (submitted via PharmCAS).
  • Actively work toward completion of all prerequisites listed in the table below.

Doctor of Physical Therapy

  • An application submitted through the Physical Therapist Centralized Application Service.
  • A letter of recommendation from a licensed physical therapist.
  • Proof of a minimum of 60 hours of observation supervised by a licensed physical therapist.

Doctor of Occupational Therapy


Occupational Therapy (OT) offers a meaningful way to help people live more independent and satisfying lives by rebuilding the skills needed for everyday activities—driving, studying, working or managing daily tasks. Occupational therapists are a critical member of the healthcare team and are in high demand nationwide with opportunities in hospitals and rehabilitation centers, schools, home-care settings and community-based programs.

Consistently ranked among the top 20 OT programs in the nation, Creighton's Doctor of Occupational Therapy Program was the nation's first entry-level OTD program. Joining the Phoenix Hybrid Pathway allows students to learn in interprofessional facilities while benefiting from the flexibility of hybrid learning.
